Insurance

We are participating in some, yet not all HMO networks. You must contact your insurance prior to your visit to verify your mental health coverage.

We will ask for your insurance information upon your initial call.  While we do our best to verify insurance before your appointment, it is your responsibility to make sure that every provider you see, is covered under your plan. 

Depending on your current health insurance provider or employee benefit plan, it is possible for services to be covered in full or in part. Please contact your provider to verify how your plan compensates you for behavioral health services and whether or not mental health claims are sent to a third party out-of-network insurance company.  A list of questions is listed below.  Please note that many insurance plans have a copayment or co-insurance that the subscriber is responsible for paying.  Estimated deductible amounts, co-insurance, and copayments are due at the time of each visit.   

 

OPTION TO SELF PAY

If you wish, we can provide you with a Superbill at the end of your appointment that you can submit to your insurance company for reimbursement. The amount you pay will initially go towards your deductible.  After your deductible is met, you will be reimbursed for at least a majority of your appointment cost.  If you have questions, please contact us. 

Forms of payment currently accepts are Visa, Mastercard, American Express, Cash, Apple Pay and Check.  Payments are due at the time of each visit.    
 

Superbills

A Superbill is simply a bill for the services you have received and paid for.  It includes the necessary information that your insurance company will need to reimburse you for your services.  It is highly recommended that you contact your insurance company to find out how to submit a Superbill for reimbursement as procedures may differ between insurance companies.   
 

Questions for your Insurance Provider

  • Does my health insurance plan include mental health benefits?

  • Is this provider covered under my plan?

  • Do I have a copay? If so, what is the amount?

  • Do I have a deductible and if so, what is it and have I met it yet?

  • Do I have co-insurance? If so, what this the percentage that I will be responsible to pay?

  • Does my plan limit how many sessions per calendar year I can have? If so, what is the limit?

  • Do I need written approval from my primary care physician in order for services to be covered?

  • How does my plan pay for an Out-of-Network Provider?

  • How do Superbills work and how do I submit them? What is the amount reimbursed for office visits?

Cancellation Policy 

We require 24 business hours notice if you need to cancel or reschedule. Please understand that your time slot is time we have reserved specifically for you. If you need to cancel or do not show up to your appointment, there will be a cancellation fee. If you need to cancel, please call us at 858.209.9871 at least 24 business hours before your appointment to avoid the cancellation fee. We appreciate your understanding in this matter.
